چکیده

Burns injuries account for over 2 million cases in our country. With over 75% of the patients sustaining injuries to the hands, post burn contractures of the finger are a very common condition presenting for treatment. Similarly trauma to the upper limbs, accounting for about a third of all injuries to the body also result in flexion deformities of the fingers. Standard treatment for these deformities is by way of surgical release of the contractures, skin cover with split skin grafting or full thickness grafts followed by prolonged immobilization and splintage. The relatively minimally invasive technique of distraction has been used in this small but representative series to correct flexion deformities caused by burns and other injuries. Results have been very gratifying.

Soft tissue distraction in hand surgery: the “pentagonal frame” technique

Soft tissue distraction (STD) is an increasingly accepted operation in all fields of hand surgery from elbow contracture release to PIP joint release. Current techniques reported lack the ability to distract the joints of the fingers or the hand, maintain the length of released contractures, and hold them in a position while active and passive physiotherapy is possible. We describe a technique ...

Gradual joint distraction of post-traumatic flexion contracture of the proximal interphalangeal joint by a mini-external fixator.

We present the outcome of the treatment of chronic post-traumatic contractures of the proximal interphalangeal joint by gradual distraction correction using an external fixator. A total of 30 consecutive patients with a mean age of 34 years (17 to 54) had distraction for a mean of 16 days (10 to 22). The fixator was removed after a mean of 29 days (16 to 40). Assessment at a mean of 34 months (...

Minimally invasive correction of phalangeal malunion under local anaesthesia.

Posttraumatic malunion of the proximal phalanx of the fingers may cause visible deformities and impair hand function. Severe dysfunction requires surgical correction, most often via open corrective osteotomies. However, such an approach requires longer bone healing time, inevitably results in a scar, and has a higher potential for extensor tendon adhesion. When performed under general anaesthes...

Correction of Long Standing Proximal Interphalangeal Flexion Contractures with Cross Finger Flaps and Vigorous Postoperative Exercises

PURPOSE We reviewed the results of cross finger flaps after surgical release and vigorous postoperative exercises for long-standing, severe flexion contractures of the Proximal Interphalangeal (PIP) joints of fingers. MATERIALS AND METHODS In 9 patients, all contracted tissue was sequentially released and the resultant skin defect was covered with a cross-finger flap. The cause of the contrac...
